Foundation Competences
Alternative Certificate A
Applicants who are not currently undertaking a UK Foundation
Programme or those who cannot provide standard documentary evidence of
recent achievement of foundation competences should ask someone who has
supervised them for at least 3 months in the last 3 years to complete
and sign an Alternative Certificate. Alternative Certificate A should
be used for posts with acute medical
responsibilities. For posts without acute medical
responsibilities, use Alternative Certificate B. Further guidance
regarding documentary evidence of achievement of foundation competences
is given in the Entry
Requirements pages of the Recruitment section.
This is a word document which can be either be printed and
completed by hand or it can be saved and completed using a computer,
then printed and signed. All signatures must be original. Applicants
should then scan the signed document and attach it to their
application form.
